This letter will be sent to all parents when students are placed in our structured workplace learning program.

Dear Parents/Guardians

Attached is your son/daughter’s Work Place Agreement Form which will provide insurance cover for them whilst on structured work placement.  The insurance cover is simply transferred from school to their work placement.  Following is an extract from Education Queensland about the insurance.

Education Queensland and Queensland Catholic Education Commission  insures students from schools under a Contract of Insurance against impairment or death, occurring at the location or another location where work experience is provided or while travelling directly between their home or school and the site where the placement is provided.

The insurance does not cover payments for medical or hospital treatment, for rehabilitation or for artificial limbs or the cost of ambulance transport.  Parents requiring medical or hospital cover or personal accident insurance for their children should make private arrangements.

So that work placement can be processed effectively, students have been advised of the following process before their work placement commences. 

Students are asked to:
a Sign the insurance form, together with you as a parent/guardian.
a Telephone the employer to make an appointment to meet with them at least one week before their placement date.
a Attend an interview to ask their provider to sign their Work Experience Agreement Form (insurance form). 
a Find out certain details during the interview such as: start/finish times, dress requirements – including any special health and safety clothing. 
a Leave the Induction Checklist and the student placement details with the work place provider.
a Return the signed Work Experience Agreement Form (insurance form) back to the Principal.

It is important that students make the telephone call and attend the interview as this is an essential part of them becoming more confident and gaining worthwhile employability skills.  Students are asked to be appropriately dressed for this interview as the first impression of your son/daughter – from the employer’s perspective - will be important.

If for some reason the student is unable to attend their placement, it is the responsibility of the student to contact the employer and advise them of the situation at least one week prior to commencement of placement.  If there is a change to this placement an  ADDITIONAL  FEE will apply.  If the employer is NOT contacted by the student, and the placement is cancelled for whatever reason by the school or the student, a CANCELLATION  FEE will also apply.
